APPLICATION FORMS

We would recomend that you attend an Open Day before completing the application forms, to attend an Open Day, click here. 

If you would like to apply to join our Learning and Skills Department at Portland College we will need you to complete the following forms:

1. Education Department Initial Assessment Document
2. Personal & Healthcare needs initial Assessment Document

 

Why do we need these forms?

We appreciate that the forms are quite in depth but it is important that they are filled out in detail to ensure that we have as much information as possible about each student before they cpme for an assessment at Portland College. This will help us prepare for their need when they arrive for example making sure we have enough staff allocated.

PLEASE NOTE: These application forms are only for students wishing to join the Learning and Skills Department. Anyone wishing to join the Employment Department should contact their local Disability Employment Advisor (DEA) at JobCentre Plus, to be forwarded to the JobCentre Plus website click here.
